Good morning.
Are any Probate Courts willing to share their forms used in mental illness cases?
I would like, if possible, to have all forms that the Clerk uses (notice of hearing, notice of rights, etc.) as well as all Court Orders that Judge would issue, i.e. initial hearing orders, detention order, appointment of examiner order, full hearing order - 90 day commitment, continued commitment orders, forced medication orders, or any other type of order that may come up as needed in this type of case.
Also, if you have a Clerk and/or Judge Checklist/Guide that you use, that would be helpful too. We refer to the Probate Bench Cards on civil commitment, but would also like to see how other Courts handle these types of cases.
